INFORMED CONSENT
I, the undersigned, hereby expressly and affirmatively state that I am willingly participating in the Virtual and Online Sessions and face to face gym sessions by Allison Everett and employees and contracted trainers of Fitquest360.
I realise that my participation in these activities involves potential risk of injury, including but not limited to bodily injury, heart attack, stroke or even death. I also recognise there are other risks associated with exercise and personal training and that it is not possible to list every one.
I know and understand the risks of exercise. Understanding that injuries are a possibility, I hereby assume all risks of injury that could occur by reason of my participation in this training program.
WAIVER AND RELEASE OF LIABILITY
I, the undersigned, intending to be legally bound, and recognizing the danger involved in physical exercise, do agree as follows:
In consideration for the services rendered by Allison Everett and employees and contracted trainers of Fitquest360 in the provision of Group Training, I agree to waive any rights, claims or damages for injuries that may occur as a result of my participation in said program.
I agree to disclose any physical limitations, disabilities, ailments or impairments that may affect my ability to participate in said program.
I understand that Allison Everett – Fitquest360 is a personal & group training and online fitness business and not a medical doctor, and that it will, in fact, be relying on my representations and disclosures regarding my health and physical condition.
I agree to have my photo taken during the session, and to be used for current and further marketing through Facebook and other social media/marketing channels.
I also do not hold Allison Everett and employees and fitness contractors of Fitquest360 liable for any personal injuries, bodily injuries or property damage sustained while undertaking the training and/or nutrition program.
